NCSI is licensed by the Minnesota Department of Human Services to provide services to people with disabilities and those over age 65 under Minnesota Statutes, Chapter 245D, Home and Community-Based Services Standards.

We develop and follow program policies to maintain compliance with 245D standards and ensure the dignity, health and independence of the people we serve. These policies are updated if statute revisions are made during a legislative session.

NCSI COVID-19 Protocols

If a person served by NCSI tests positive for COVID-19 or is in close contact with someone who has tested positive for COVID-19, please contact executive director Jennifer Freeburg immediately at jennifer.freeburg@NCSIdaysupports.org or 651-636-3343 ext. 101 to receive the most current protocols for staying home.

NCSI COVID-19 Preparedness Plan

Please review our preparedness plan, which includes participation requirements, before making the decision to participate in NCSI’s on-site services; download a copy of NCSI’s COVID-19 Preparedness Plan (PDF).

We are serving people in consistent cohorts, and the staff wear masks when serving people. People served must wear masks while riding transportation; while on-site, people served are strongly encouraged but not required to wear masks.

People considered by the CDC to be at higher-risk for COVID-19 are still strongly encouraged to stay home per the Governor’s “stay safe” order.

NCSI COVID-19 Waiver of Liability

Prior to a person participating in NCSI’s on-site services, her/his legal representative must sign and submit our permission form; download a copy of NCSI’s COVID-19 Waiver of Liability (PDF).

NCSI COVID-19 Screening Checklist

Every day before coming to NCSI, people served and their parents/guardians/residential service providers are required to self-monitor for signs/symptoms of COVID-19 using our screening checklist; download a PDF copy of NCSI’s COVID-19 Screening Checklist.
